Services: Walk in My Shoes (WIMS) Social Prescribing Program

Objective:
To enhance the well-being of individuals in Black and Racially Minoritised (BRM) communities through a holistic Social Prescribing approach, with a focus on mentoring programs.

Overview: Social Prescribing is a proactive approach that recognizes health and well-being as interconnected with various aspects of life. For WIMS, this involves linking individuals in BRM communities to tailored non-clinical activities, providing practical and emotional support. The program seeks to address concerns, foster community connections, and empower individuals on their health journey.

The Social Prescribing Approach for WIMS:

Identification of Concerns:

Participants in BRM communities, referred to as 'Persons,' express concerns, which may include issues like loneliness, mental health, or specific challenges.
Professional Referral:

Participants are referred to the WIMS Social Prescribing Program by healthcare professionals or community advocates. This referral acknowledges that non-clinical support could significantly contribute to the individual's well-being.
Personalized Intervention:

Each participant, upon referral, is connected with a tailored social prescribing intervention, primarily the WIMS mentoring program. This may involve one-on-one mentoring, group activities, or participation in community-led initiatives.
GP Collaboration:

WIMS collaborates with General Practitioners (GPs) to ensure a holistic approach to health. GPs are informed about the non-clinical interventions, and medical concerns raised by participants are addressed through appropriate healthcare channels.

Three Components of the Social Prescribing Approach for WIMS:

  • Individual Expression of Concerns:

    • Participants openly share their concerns, creating a comprehensive understanding of their needs.
  • Professional Referral and Prescription:

    • Healthcare professionals prescribe participation in WIMS programs, recognizing the potential benefits of non-medical interventions.
  • Personalized Support and Collaboration:

    • WIMS provides personalized support through mentoring programs and collaborates with healthcare providers to ensure a comprehensive approach to well-being.
